Application Forms

In addition to completing an application form for the Graduate School at NIU, the Professional M.B.A. Program requires a completed supplemental application (please note: the $30.00 application fee is to be submitted with the graduate school application form), college transcript, letter of recommendation, and sponsor agreement.
